Service Description
At our practice, we offer medicinal cannabis evaluation to those seeking alternative treatment options. Our practitioner is licensed and registered with the Medicinal Cannabis Program and will assess your medical condition and provide a recommendation if this treatment plan is right for you. Important: If you are not an established client with Modern Integrative Psychiatry, medical records with a qualified diagnosis from your provider are REQUIRED before appointment approval. If you do not have a primary care provider (PCP) or records, establishing care with Modern Integrative Psychiatry is required before an evaluation.
Cancellation Policy
Cancellations or rescheduling appointments must be done 24 hours in advance if you are unable to keep your new client visit or follow-up visit. Late Cancellations, No Shows, and Late Rescheduled sessions will be charged a fee for each late cancellation/no show/rescheduled appointment if NOT RECEIVED AT LEAST 24 HOURS IN ADVANCE.
